Closes the six non-blocking review comments deferred out of #338 (three Should Fix, three Nice to Have).

1 — the secret scan is probed, not described (comment 2)

reflect --non-interactive writes KB pages with no operator reviewing the learning list, so this scan is the only thing between the conversation and the vault. Prose could not carry it: a reworded list reads identically whether the scan works or not, and the merged text was both over- and under-broad at once.

scripts/reflect-secret-scan.sh is now the single executable source, and scripts/reflect-secret-scan-test.sh probes it in both directions — each secret shape rejected, each innocent string kept. One direction alone passes for an accept-all scan, the other for a reject-all one; together they distinguish a working scan from a broken one.

  • sk- is anchored to a token boundary — task-, risk- and disk- no longer match
  • the password= / token= / secret= / api_key= shapes require an assignment with a value of 8+ characters, so prose about a token= field does not fire
  • JWT and base64-blob shapes are added; the base64 class excludes pure hex, so a git SHA is not mistaken for a blob
  • commands/reflect.md keeps the class list as the fallback for an unreachable plugin cache, and a pin derives that list from the script's own --classes output, so the two cannot drift

2 — the mixed terminal line (comments 3 and 6)

reflect: wrote <N> page(s): [[<a>]]; skipped <M> — sensitivity — one line recording both, so a sensitivity skip is no longer lost the moment the write is reported. session-close Phase 9.4 records the same vocabulary. The changelog bullet enumerates all four forms, reflect: failed — <reason> included; the released ## v0.181.0 section is left untouched.

3 — the two session-close prose collisions (comments 4 and 5)

The refused-exit carve-out now states that it wins over the goal-anchored block's ⏰ Next: line, with the combined form given so the goal's next task is kept. The mode-3 advisory tail's "never auto-run" is scoped to a mode-3 session, so it no longer contradicts Phase 9.4's mode-2 auto-run of reflect.

4 — the Phase 9.4 rule is pinned (comment 1)

scripts/mode-membership-rule-test.sh now pins the rule itself — reflect runs in mode 2 only, and only when both the Phase 8 reflect signal fired and a finding names no existing KB page — with a mutation self-check that removes each of the three conjuncts in turn and requires that conjunct's pin to fail. Every earlier check pinned a consequence of the rule and none pinned the rule, so a dropped clause or a widened scope left them all green.

Verification

  • make precommit — pass
  • scripts/mode-membership-rule-test.sh — 46 passed, 0 failed (22 before)
  • scripts/reflect-secret-scan-test.sh — 16 passed, 0 failed
  • mutation controls against the real file: removing each Phase 9.4 conjunct fails exactly its own pin (45 passed, 1 failed, three times)

Outstanding

The invocation exercise is not run. commands/*.md cannot be exercised by the session that authors them. Once this merges and the plugin is updated: the operator runs /reload-plugins, then a reloaded session runs /vault-cli:reflect --non-interactive against the two-learning sample (one clean, one fake credential-shaped fixture) and quotes the terminal line it prints.

reflect --non-interactive writes KB pages with no operator reviewing the
learning list, so its sensitivity scan is the only thing between the
conversation and the vault. Prose could not carry that: a reworded list
reads identically whether the scan works or not, and the review found the
merged text both over- and under-broad at once — a bare 'sk-' matched
'task-', 'risk-' and 'disk-', while JWT and base64 shapes matched nothing.

- scripts/reflect-secret-scan.sh is now the single executable source of
  the pattern set, and scripts/reflect-secret-scan-test.sh probes it in
  BOTH directions: every secret shape rejected, every innocent string
  kept. One direction alone passes for an accept-all scan, the other for
  a reject-all one. The base64 class excludes pure hex so a git SHA is
  not mistaken for a blob; the assignment shapes require a value, so
  prose about a 'token=' field does not fire.
- reflect's NO-ASK terminal line gains a mixed form, so a run that wrote
  one page and skipped one for sensitivity records both in one line
  instead of losing the skip.
- session-close resolves two contradictions: the refused-exit carve-out
  now states it wins over the goal-anchored block's Next line, and the
  mode-3 advisory tail's 'never auto-run' is scoped to a mode-3 session.
- mode-membership-rule-test.sh pins the Phase 9.4 rule itself, with a
  mutation self-check that removes each of its three conjuncts in turn
  and requires that conjunct's pin to fail.
The `while read … done < <(script --classes)` form fails OPEN: a script
that errors or is gone produces no input, the loop body never executes,
zero checks run, and the file reports a clean run having pinned nothing.
Capture the list first and assert its length, so a missing script fails
the pin instead of retiring it. Probed: removing the script now reports
'want [10], got [0]' and fails.

Four Should-Fix findings, all real:

- reflect.md never assigned $REFLECT_SCAN. The prose gave only the
  directory command, so a literal reading passed a directory to bash and
  the invocation failed into the prose fallback — which reads as a
  working scan. Show the full assignment with the filename appended.
- the api-key class matched only sk-, so Stripe sk_live_/rk_live_ keys
  scanned clean. Add stripe-key and name it in the fallback list.
- bearer-token was case-sensitive, so 'authorization: bearer <token>' —
  an equally valid form, HTTP auth schemes being case-insensitive —
  scanned clean. Match the scheme in any casing.
- nothing asserted that a rejected verdict omits the matched value, so an
  edit that echoed the match would keep every check green while putting
  the secret into the transcript the scan exists to keep it out of.

Also: an unreadable stdin failed open (empty text scans clean); a quoted
assignment value containing spaces never matched; and the class-name pin
grepped all of reflect.md rather than the fallback paragraph the model
actually applies when the script is unreachable.

…tions

Two real defects, both found by the bot:

- the no-leak assertion was a tautology. 'scan ... | grep -F hunter22'
  cannot carry it: scan exits 1 on a rejection, so under 'set -o pipefail'
  the pipeline is non-zero whether or not grep matched — the '&&' branch
  was unreachable and the check passed on a scan that echoed the secret
  in full. Capture stdout first, then grep the string.
- key-assignment matched only bare lowercase keys, so the dominant config
  shapes — PASSWORD=, API_KEY=, client_secret=, access_token= — scanned
  clean. Match the key name case-insensitively and allow a prefix. The
  cost is that prose like 'the access_token= parameter' can fire; that is
  the safe direction, since a false positive skips a learning while a
  false negative publishes a secret.

Also: GitLab glpat- and npm_ tokens added; an unreadable stdin now has a
fixture (a directory on stdin); the class count became a floor so adding
a class is not a four-site edit; and the fallback extraction now ends on
the next paragraph's opening with a length assertion, so a reworded
closing sentence cannot run it to EOF and re-create the whole-file false
green it exists to prevent.

Probed: echoing the match fails 19 checks, reverting the stdin guard
fails 1, reverting the key pattern to lowercase-only fails 2.

Merging origin/master was CLEAN, and that was the failure: git folded this
branch's four Unreleased bullets into the already-released v0.182.0
section, with no conflict marker and no heading change to notice. The
placement assertion is what caught it (mine read 'sits under: ## v0.182.0').

Neither this branch's merge nor its work commit is in any tag, so the
repair direction is 'back under ## Unreleased', never into a released
section. Master's own 'feat:' bullet stays where it belongs in v0.182.0.

Verified after the repair: mine under ## Unreleased, master's under
## v0.182.0, heading diff exactly '+## Unreleased', and the ^--[^-]
completeness probe shows nothing dropped.

The CRITICAL was mine: the changelog pin asserted '## Unreleased', but this
repo is autoRelease:true and the releaser RENAMES that heading, so the
extraction goes empty and the pin would have failed 'make test' on master
for every commit after the next release — a test I added would have broken
the build. Scope it to the newest section whichever it is, and add the
durable half against commands/reflect.md, which is the contract; the
changelog bullet is a one-time record of it. Proven by simulation: renaming
the heading to '## v0.185.0' in a copy leaves the new pin green while the
old expression finds nothing.

Also:
- REFLECT_SCAN could resolve to /reflect-secret-scan.sh, where bash exits
  127 printing nothing and 'no rejected line' read as clean. Add an [ -x ]
  guard and state that no output means the scan did not run.
- key-assignment required the secret word to END the key, missing
  AWS_SECRET_ACCESS_KEY= and secret_key=; connection-string required a
  non-empty user, missing redis://:pass@host.
- every fixture compared stdout only, so a scan that printed 'rejected' but
  exited 0 passed. Assert the exit contract in both directions.
- name the line-oriented gap beside the base64url one in KNOWN LIMITATIONS.

The commit's simulation covered the release rename (`## Unreleased` → `## v0.185.0`, same body, still green) but not the step after it: b86bbaf shows the releaser renames in place, so the section disappears and the next PR recreates an empty-of-this-bullet `## Unreleased` at the top — which is exactly the documented workflow in CLAUDE.md and what 0f982dc did after v0.184.1. The result is a test that breaks the build for the next person. The durable half of this pin already exists — the `reflect: failed — <reason>` check against `commands/reflect.md` (line 235) — so the changelog pin should be dropped, or scoped to the fixed historical version it was written for, rather than to whichever section is newest."

One Should-Fix, non-blocking finding: the `base64-blob` class rejects any 40+ character run of the base64 alphabet that is not pure hex, which includes ordinary slash-delimited paths (e.g. `claude/plugins/marketplaces/coding/docs/changelog`, 49 chars) — a realistic learning text, so a legitimate learning is silently skipped. The class's own stated rule (`scripts/reflect-secret-scan.sh:90-94`) claims it matches \"what a random key encodes to, and what a SHA does not\", but a path matches too. The test suite probes the SHA false positive (`scripts/reflect-secret-scan-test.sh`) yet has no path-shaped KEPT fixture, so this passes both directions. Round 3's repair was still wrong, one step further out. Scoping the pin to
the newest section only holds until the next PR opens a fresh
'## Unreleased' above the renamed one. So: pin '## Unreleased' -> dies at
the release; pin the newest section -> dies at the PR after it. Both were
written here and both would have failed make test on master — a test added
to guard the change, breaking the build once the change shipped.

A changelog bullet is a one-time record and has no durable property to pin.
Delete the pin; the contract lives in commands/reflect.md, whose
'reflect: failed — <reason>' pin is the durable half.

Also: the three Phase 9.4 mutation controls now call one phase94_pin
function — the pin's OWN expression — instead of restating its grep, so a
later loosening of the real pin cannot leave its control green; and
session-close.md's terminal-line enumeration gains the fourth form,
'reflect: failed — …', which the paragraph below it already records.

Re-proved the three real-file mutations after the refactor: each still
fails exactly its own pin.

  "summary": "The script/pin/prose hardening is internally consistent: the new class-sync pin, the Phase 9.4 decision-rule pins and their mutation controls, and the session-close precedence text all resolve against the worktree, and the new script test probes both directions. The one substantive gap is that the new reflect.md scan snippet never assigns `$LEARNING_TEXT`, so a literal run feeds empty stdin and the scanner reports `secret-scan: clean`; two documentation nits are non-blocking. Selector traceability: 20 candidates, 8 applicable, 12 skipped; mechanical funnel 0 findings.",

The surrounding \"Silence is not a clean verdict\" guard covers no-output and non-0/1 exits, not an empty-input `clean` line, so the learning is written unscanned. Add the assignment (e.g. `LEARNING_TEXT='<the drafted learning text>'`) or an explicit instruction to set it, and state that empty input scans clean."

Verified by grep that this file's fixtures match the gitleaks-derived patterns trivy ships: `glpat-[0-9a-zA-Z_-]{20}` (line 66), `npm_[a-zA-Z0-9]{36}` (68), `xox[baprs]-…` (72), the JWT `eyJ….….…` (109), and the `-----BEGIN RSA PRIVATE KEY-----` header (60). Note the Plan's named fixtures are actually safe: `AKIAIOSFODNN7EXAMPLE` matches the `example` allow rule, the `ghp_` body is 30 chars (< trivy's 36), and the `sk_live_` body is 20 chars (< trivy's 24); it is the other fixtures that are exposed. Chunk 1/3

  "verdict": "request-changes",
  "summary": "The pre-computed mechanical funnel returned zero findings; the review below is the in-session judgment tier over this chunk (CHANGELOG.md, Makefile, commands/reflect.md, commands/session-close.md, scripts/mode-membership-rule-test.sh). The added test pins are sound and match the current prose, and the Makefile wiring and version alignment are fine. Two prose defects remain in the reflect/session-close contract: the NO-ASK scan snippet's single-quoted `$LEARNING_TEXT` is not quote-safe, so a learning containing an apostrophe can leave the scanner printing `secret-scan: clean` for text it never scanned (blocking, since this is the exact failure the PR exists to remove), and the new goal-anchored refused-exit panel still prints `session closed` while asserting the pane stays open.",

With an apostrophe in the text, bash closes the quote early and either (a) parses the remainder as a command, so the assignment does not persist and `$LEARNING_TEXT` keeps the PREVIOUS learning's value in the same shell, or (b) parses the remainder as assignments only, persisting a truncated prefix. In both cases the scanner runs on text that is not the learning and prints `secret-scan: clean`, and under NO-ASK that learning is written — a credential leak via a scan that never inspected the text, the exact failure the PR claims to close.",
      "message": "Should Fix (Important): the snippet assigns the learning text in single quotes, so an apostrophe breaks the assignment. Because the prose says the scan runs once per learning (multiple learnings in one shell), a misparsed second assignment leaves `LEARNING_TEXT` holding the first learning's text and the scanner reports `secret-scan: clean` for a learning it never read. Use a quoted heredoc / stdin instead, e.g. `LEARNING_TEXT=$(cat <<'EOF' … EOF)` with the scan fed from a file, and reset the variable (`LEARNING_TEXT=`) before each learning so a failed assignment can never inherit the previous value. The `[ -n … ]` guard catches only the empty case, not a stale or truncated value."

The combined form should drop `— session closed` (e.g. `👤 You: nothing — exit refused`) so the panel is internally consistent when the exit was refused. "detail": "`unreadable-input` is an I/O status, not a sensitivity class, and is emitted only when the script actually runs; the prose fallback applies only when the script is unreachable, so no equivalent is needed. `--classes` correctly omits it and the sync pin therefore does not require it in the prose."
    },{
      "concern": "correctness: `if ! text=$(cat)` fails closed only when cat exits nonzero; redirection from /dev/null succeeds with empty text and scans clean, which the test asserts deliberately. Confirm the split contract between script and reflect.md is intentional and consistent.",
      "disposition": "not-an-issue",
      "detail": "Intentional split: the script rejects an unreadable stream; an empty stream is clean by design, and reflect.md's `-s` guard plus 'silence is not a clean verdict' clause make the caller responsible for never feeding an empty file. The test asserts the empty case explicitly (line 137)."

Verify all four exist verbatim in the head commands/session-close.md, or the new checks fail.",

      "concern": "correctness: the FALLBACK awk extraction anchors on `Fallback when the script is unreachable` and ends at the indented `The Phase 3 filename list` line, with a 20-line ceiling. Verify indentation and anchors match reflect.md exactly and that the block is one paragraph.",
      "disposition": "not-an-issue",
      "detail": "Verified: reflect.md:44 begins with two spaces before `**Fallback when the script is unreachable**`; reflect.md:48 begins with exactly two spaces before `The Phase 3 filename list`; the extracted block is 4 lines (≤20) and names every class in backticks."

      "disposition": "not-an-issue",
      "detail": "`between` prints the start-anchor line and exits before printing the end-anchor line. Phase 9.4 = line 755, Phase 9.5 = line 771; the mixed literal occurs at line 765 (inside) as well as line 719 (outside), so the in-range occurrence satisfies the pin."

      "concern": "tests: the kept direction omits near-misses for private-key and connection-string (for example a plain https URL and prose about a PEM header). Check the kept fixtures exercise the anchor itself rather than passing on length.",
      "disposition": "not-an-issue",
      "detail": "The positive fixtures exercise both anchors (private-key requires the literal `-----BEGIN … PRIVATE KEY-----`; connection-string requires `scheme://user:pass@`). The kept direction lacks those specific near-misses — a minor coverage enhancement, not a defect; the test file is outside this chunk."
    },{
      "concern": "tests: `scan()` discards stderr and `scan_status()` relies on `echo $?` after a pipeline under pipefail. Verify the exit-code probes measure the script rather than pipeline artifacts, and that a script error is not swallowed into a false clean.",
      "disposition": "not-an-issue",
      "detail": "`scan_status` runs `printf | bash \"$SCAN\" >/dev/null 2>&1` and then `echo $?` as a separate command, so it reports the pipeline status. Under pipefail the scanner's exit code dominates (printf succeeds), and a script error exits non-zero, failing the 1/0 assertions rather than being swallowed."

      "disposition": "not-an-issue",
      "detail": "The test resolves `ROOT=$(cd \"$(dirname \"$0\")/..\" && pwd)` and `cd`s there; it reads only its own fixtures and the scan script and carries no cross-script state. The Makefile invokes it as `@bash scripts/reflect-secret-scan-test.sh`."

      "concern": "docs: the REFLECT_SCAN resolver appends the filename to the newest plugin-cache scripts directory; when the glob matches nothing the path collapses to /reflect-secret-scan.sh. Verify the executable guard and the prose fallback are reachable, and that the mixed terminal line is defined for the fallback path too.",
      "disposition": "not-an-issue",

The mixed terminal-line vocabulary is defined independently of the scan path."

      "disposition": "not-an-issue",
      "detail": "session-close.md:838 is the refused-exit goal-anchored panel (`nothing — exit refused`); the remaining `nothing — session closed` occurrences (lines 800 and 830) are the non-refused mode-2 and goal-anchored cases. The carve-out at line 816 states precedence and gives the combined `⏰ Next:` line; the later block defers to it."
