Move the pinned-ID write out of the render-phase useMemo.
usePinnedViewedItem serves both permissions and forms. It mutates pinnedIdRef.current during render. React 19.2.3 may replay or discard render work, so a discarded render can persist a pin for an item that was not committed. A later committed render can then select that item instead of retaining the item that was displayed.
Use state and update the pin in an effect:

🪛 React Doctor (0.9.14)
[error] 116-116: This ref is mutated during render. React can replay or discard render work, so the mutation can leak from UI that never commits.
Move ref writes into an event handler or effect. Render must stay pure because React can replay or discard it. The predictable null-guarded lazy initialization pattern remains supported.

Summary
The global permission dialog always showed the first pending permission across all sessions and repos, with no session or repo context. A user viewing one session could approve another session's command without knowing it.
Selection (
frontend/src/contexts/EventContext.tsx)usePinnedViewedItem.permission.askedandform.createdresolve the session's parent link when it is unknown, so subagent requests get the correct priority.0,?assistant=1). This also fixes the toolbar badge, the notifications sheet, and form navigation, which had the same silent bug.Dialog (
PermissionRequestDialog.tsx,App.tsx,shared/src/notifications/format.ts)useSession), a "From another session" warning, and an "Open session" action.message.savelist, with the OpenCode TUI wording. Hides "Allow Always" whensaveis empty, which matches OpenCode (all built-in tools sendsave).Validation
notification-format.test.ts: 37 passed.Known behavior (not changed here): a pending permission from any session still opens a modal dialog in every tab. It now states which session it belongs to. A non-blocking prompt for other sessions' requests is a possible follow-up.
